ABSTRACT

OBJECTIVES@#To study the value of basal luteinizing hormone (LH) level combined with uterine volume measurement in the early diagnosis of central precocious puberty (CPP) in girls with different Tanner stages.@*METHODS@#A retrospective analysis was performed on the girls who presented with breast development before the age of 8 years and attended the Third Affiliated Hospital of Zhengzhou University from January 2017 to September 2022. According to the results of gonadotropin-releasing hormone (GnRH) agonist test, the girls with peak LH ≥5.0 IU/L and peak LH/follicle stimulating hormone ≥0.6 were enrolled as the positive group, and the other girls were enrolled as the negative group. The two groups were compared in terms of the basal LH level and uterine volume. The receiver operating characteristic (ROC) curve was used to analyze their value in the early diagnosis of CPP.@*RESULTS@#For the girls with Tanner B2 and B3 stages, the positive group had significantly higher basal LH level and uterine volume than the negative group (P<0.05). The basal LH level had an optimal cut-off value of 0.325 IU/L and 0.505 IU/L respectively in the diagnosis of Tanner stage B2/B3 CPP, while uterine volume had an optimal cut-off value of 1.639 mL and 2.158 mL respectively. Basal LH level combined with uterine volume measurement had a significantly larger area under the ROC curve than uterine volume measurement alone (P<0.001), but with no significant difference compared with that of basal LH level measurement alone (P>0.05).@*CONCLUSIONS@#Basal LH level combined with uterine volume measurement is valuable in the early diagnosis of CPP in girls with different Tanner stages, which provides a basis and guiding significance for clinical diagnosis of CPP.

ABSTRACT

OBJECTIVE@#To observe the clinical therapeutic effect of the combination of electroacupuncture (EA) at @*METHODS@#A total of 58 patients after uterine curettage of incomplete abortion were randomized into an EA group and a western medication group, 29 cases in each one. In the western medication group, mifepristone tablets were administered orally, 2 tablets each time, once daily. ABSTRACT

Objective To measure the uterine volume by ultrasonic imaging in nullipara patients with endometriosis and analyze its clinical significance. Methods From July 1st, 2016 to August 1st, 2017, 107 cases of nullipara patients with endometriosis hospitalized in Peking University First Hospital were selected as endometriosis group. Among 107 cases, 59 cases were in proliferative phase of menstrual cycle, 48 cases were in secretory phase; and 10 cases had an abortion history.Totally 101 cases of patients without endometriosis operated for other benign disease during the same period were chosen as the control group. All patients underwent laparoscopic surgery, and the diagnosis was confirmed by pathology. The uterine size was measured by transvaginal or transrectal color Doppler ultrasound, and the uterine volume was calculated according to the known formula. Results The uterine volume of the endometriosis group [(69±26) cm3] was larger than that of the control group [(54±18) cm3, P<0.01]. In the endometriosis group, the uterine volume of patients with pregnant history [(78±34) cm3] was larger than that of patients without pregnant history [(68± 25) cm3]. The endometrial thickness in endometriosis group was higher than that of the control group [(9.1± 3.5) versus (7.8±2.9) mm, P<0.05], and positively correlated with the uterine volume (r=0.39, P<0.05). The hemoglobin in endometriosis group was lower than that in the control group [(124±12) versus (131±10) g/L, P<0.01], and was negatively correlated with the uterine volume in the two groups (rendometriosis=-0.12, rcontrol=-0.21; both P<0.05).The uterine volume of dysmenorrhea patients in the endometriosis group [(73±28) cm3] was greater than that of patients without dysmenorrhea [(62 ± 19) cm3, P<0.01]; both uterine volumes of patients with and without dysmenorrhea in endometriosis group were larger than those of patients with or without dysmenorrhea in the control group (P<0.05). The degree of dysmenorrhea in endometriosis group was positively correlated with the uterine volume (r=0.20, P=0.042). The uterine volume of stageⅣendometriosis patients [(79±30) cm3] was greater than that of stage Ⅲpatients in endometriosis group [(58±14) cm3, P<0.01]. Conclusion Patients with endometriosis are of increased uterine volume, which may be related to dysmenorrhea, menorrhagia and infertility.

ABSTRACT

Objective To investigate the effect of Huoxue-Xiaozheng decoction on ovarian function and mifepristone of in patients with uterine leiomyoma after operation.Methods A total of 90 patients with uterine leiomyoma after operation were divided into the observation group and control group according to random number table method,with 45 cases in each group.The control group were treated with mifepristone,and the observation group with Huoxue-Xiaozheng decoction on the basis of the control group.The serum sex hormone (FSH,LH,E2,P and T),cell factor (TGF-β3,IL-22,FSH,LH,E2,P and T) and the uterine volume between two groups after treatment were compared,and the recurrence rate of uterine leiomyoma was compared between the two groups after treatment.Results After treatment,the serum sex hormone FSH (17.11 ± 4.01 μg/L vs.20.16 ± 5.21 μg/L,t=3.112),LH (16.78 ± 3.34 μg/L vs.19.52 ± 4.32 μg/L,t=3.366),E2 (156.60 ± 46.32 pmol/L vs.220.31 ± 50.11 pmol/L,t=6.263),P (8.37 ± 2.50 μg/L vs.15.23 ± 2.45 μg/L,t=13.1467),T (0.23 ± 0.05 ng/ml vs.0.31± 0.06 ng/ml,t=6.8712),cell factor TGF-β3 (22.08 ± 4.34 μg/L vs.28.10 ± 5.02 μg/L,t=6.086) of the observation group were significantly lower than those of the control group (P<0.01).After treatment,the uterine volume (85.42 ± 19.35 cm3 vs.97.37 ± 20.45 cm3,t=2.847) of the observation group was significantly lower than that of the control group (P<0.01).After 2 years of follow-up,the uterine fibroids overall recurrence rate of the observation group was 13.3% (6/45),while the control group was 31.1% (14/45),.The difference between two groups,was statistically significant (x2=4.114,P=0.043).Conclusions The Huoxue-Xiaozheng decoction combined with mifepristone can effectively inhibit the secretion of ovarian hormones and decrease the expression of TGF-β3,IL-22,FSH,LH,E2,P and T in patients with uterine leiomyoma.
